Interlibrary Loan Request

Form for requesting an item from another library through the Perry Public Library interlibrary loan service. Please note that beginning July 1, 2009, a $1.00 fee per item will be assessed to your library account for interlibrary loans you request.

Type author of book or magazine article.
Type title of book or magazine article.
Type the title of the journal or magazine title (for articles only). Leave blank if you are requesting a book, video, or audio title.
Supply copyright or publication year for books, audios, or videos. Supply month and year for magazine articles.
Provide ISBN, if known.
Supply volume, number, and pages for magazine articles. For all other material types, leave blank.
Supply special instructions (specific edition, format (cassette, CD, VHS, DVD, language, etc.)
